Analysis of time to recurrence in the ATAC (arimidex, tamoxifen, alone or in combination) trial acc
Background: The ATAC trial randomized 9366 postmenopausal women with early breast cancer to 5 years of treatment with anastrozole (A) or tamoxifen (T) or the combination (C) of A+T. After 47 months median follow-up the hazard ratio (HR) for disease-free survival was 0.86 (0.760.99, p=0.03) in the overall population and 0.82 (0.700.96, p=0.014) in the estrogen receptor (ER) and/or progesterone receptor (PgR) positive group, in favor of A when compared with T.

Ductal carcinoma in situ (DCIS)
Treatments and drugs
Treatment of DCIS has a high likelihood of success, in most instances removing the tumor and preventing any recurrence, particularly a recurrence that spreads beyond the original site. The challenge is to avoid either overtreating or undertreating the condition.
Treatment options for DCIS include:
- Lumpectomy only
- Lumpectomy and radiation therapy
- Lumpectomy and the drug tamoxifen
- Simple mastectomy
Surgery If you're diagnosed with DCIS, one of the first decisions you'll have to make is whether to treat the condition with breast-conserving surgery (lumpectomy) or breast-removing surgery (mastectomy).

Background In hormone-dependent breast cancer, five years of postoperative tamoxifen therapy — but not tamoxifen therapy of longer duration — prolongs disease-free and overall survival. The aromatase inhibitor letrozole, by suppressing estrogen production, might improve the outcome after the discontinuation of tamoxifen therapy.

Stage 0 (carcinoma in situ)
There are 2 types of breast carcinoma in situ:
* Ductal carcinoma in situ (DCIS) is a noninvasive, precancerous condition in which abnormal cells are found in the lining of a breast duct. The abnormal cells have not spread outside the duct to other tissues in the breast. In some cases, DCIS may become invasive cancer and spread to other tissues, although it is not known at this time how to predict which lesions will become invasive.
* Lobular carcinoma in situ (LCIS) is a condition in which abnormal cells are found in the lobules of the breast. This condition seldom becomes invasive cancer; however, having lobular carcinoma in situ in one breast increases the risk of developing breast cancer in either breast.

Adjuvant Therapy for All Patients With Breast Cancer?
In this issue of the Journal, Fisher et al. (1) from the National Surgical Adjuvant Breast and Bowel Project (NSABP) provide a retrospective analysis of the prognosis and treatment of patients with tumors 1 cm or less and negative axillary lymph nodes. They address a critical question about adjuvant therapy decision making: whether or not these therapies should be applied to patients with tumors smaller than 1 cm.

Almac Diagnostics has announced a major study analysing ductal carcinoma in situ tissue samples using its novel Breast Cancer DSA™ microarray. DSA™ research tools focus on the transcriptome of an individual disease, in this case breast cancer, and contain significant additional data, relevant to the disease of interest that is not available on other generic microarrays. The study will be conducted in collaboration with Prof Adrian Harris, Cancer Research UK and Professor of Medical Oncology at the University of Oxford.

Biological Behavior of Human Breast Cancer Micrometastases
Purpose: Clinically undetectable micrometastases may account for disease recurrence in breast cancer patients after variable disease-free intervals. However, little is known about the cellular mechanisms controlling human breast cancer micrometastases. We compared tumor proliferation rate, apoptotic index, and angiogenesis in human breast cancer micrometastases with those of macroscopic axillary lymph node metastases.
